Mannequin 5 Guide to Voting on the Website in English

1.   http://www.tbs.co.jp/arashi-chan/
2.   Scroll down a bit and click on the big blue ‘WEB投票はコチラ！’ button.
3.   Click on the pictures to enlarge. Select your favorite.
4.   Fill in the fields as follows (Starred fields are compulsory):
 

5.   On the second page, the first button is ‘Send’ and second is ‘Edit’.

You're done! Who do you think chose which outfit? :D

*Edit:
It's not necessary to fill in the 5 comment boxes.
You can fill the rest in English. Apparently postal code needs to be all numbers. (The information doesn't have to be true)
For the Kana name boxes, it's supposed to be the reading of Japanese Kanji names. Since we don't have those I suppose you could just repeat your name?
